There are many challenges facing lesbian, gay, bisexual or transgender people immigrating to the United States. Aside from the non-acknowledgement of bi-national LGBT couples, there are difficulties for immigrants with HIV/AIDS, those seeking political asylum because of their sexual orientation or those wanting to establish permanent residency in the U.S., but come from a country where lesbian and gay identity or same-sex sexual activity is considered a criminal offenseorg.

Breast Cancer
Breast cancer is a great concern in the lesbian, gay, bisexual and transgender community. According to the Mautner Project, an organization dedicated to advocacy and education around lesbian healthcare issues, particuarly those with cancer:
- Lesbians are less likely to seek health care because of the discomfort of coming out to health care providers.
- Lesbians are less likely to visit a doctor for routine gynecological services such as birth control and pre-natal care. Therefore, lesbians are less likely to have cancers detected at earlier, more tangible, stages.
- Lesbians are at higher risk of breast, cervical and ovarian cancers because they are less likely to have children by age 30, if at all.
- Lesbians are affected more directly by women's lower earning power and do not have the benefit of a spouse's health insurance coverage.

Laying the groundwork for the political and legislative movement is the Gay & Lesbian Alliance Against Defamation (GLAAD) and its brand of social activism. The organization bases its work in fighting defamation in the media, where mainstream society form their opinions of the community. The coming out of television character Ellen to the anti-gay tirades of radio talkshow host Laura Schlessinger, GLAAD has worked on it all.
